Dear Friends & Neighbors,

Greetings from Portland Habilitation Center Northwest. For those of you who might be unfamiliar, Portland Habilitation Center Northwest (PHCNW) is a local nonprofit whose mission is to train and employ people with disabilities. We recently celebrated 60 years of teaching, training, and employing underserved members of our community.

We are currently exploring a new educational opportunity. PHCNW has established a steering committee comprised of teachers, business owners, and community members to explore the creation of a new charter school located in Southeast Portland. We are considering proposing a middle school, grades 6 – 8, that aims to improve academic achievement through hands on learning curriculum. The school would utilize the latest learning technologies available, employ proven academic intervention strategies, and diligently work to establish community and family partnerships.

As we collaborate with Portland Public Schools and explore the best course of action, we would sincerely appreciate if you would fill out this short educational survey. The information we gain from you will greatly assist us as we determine the best way we can help our community.
